Small fibroid uterus

Small fibroid uterus? Or is an adenomyoma?

Ultrasound images of the uterus show a small isoechoic mass with thin borders near the endometrial stripe in a 45 year old female. 

Most likely a developing fibroid. Adenomyoma is less likely at this age. 

Final diagnosis: fibroid of uterus.

This patient had a D&C procedure some time ago. Ultrasound and color Doppler revealed an area of increased vascularity with a mesh of vessels in posterior uterine wall. 

This appearance of focal increased vascularity is suggestive of an AVM or arterio-venous malformation. In young women, the commonest cause is post procedural.

Two retroplacental hematomas in 2nd trimester pregnancy

This 2nd trimester pregnancy has an anterior placenta on ultrasound imaging. 

Patient had a history of bleeding and pain. 

But look at the hypoechoic collection in the lower part of placenta:

It's small, but the ultrasound image above suggests a retroplacental hematoma 👆.

Let's have a look at the upper margin of the placenta: see ultrasound images below:

Another area of retroplacental hemorrhage. Color Doppler ultrasound shows no flow in area. 

Rules out a mass or venous lake.

Mesenteric cyst epigastric region: sonography

A cyst in epigastric region, not vascular on color Doppler ultrasound 👆👆

Is it a pancreatic pseudo cyst? 

The pancreas appears normal on ultrasound 👆👆

The mysterious cyst is 2.5 cms in size. 

Final diagnosis: mesenteric cyst 

Advised CT scan confirmation 👍.

Multiple right VUJ calculi

Rt VUJ calculi, there are 2 of them. 

A poor twinkle artefact present 👆👆

Right ureteral jet of urine visualized 👆 suggesting partial or little obstruction.

Let's have a look at the right kidney and ureter. 

Right hydronephrosis and hydroureter hydroureter, both mild👆👆

Partial obstruction confirmed.

Large ventral hernia in adult female

This middle aged female had a large mass in anterior abdominal wall. Ultrasound imaging showed a large ventral hernia of 7 x 5 cms size. 

The abdominal wall defect measured 2 cms.

The ultrasound images below were taken with both convex and linear high frequency probes.

Fetal heart echocardiography at 20 weeks

One method I use for fetal echocardiography using color Doppler ultrasound:

• Zoom the fetal heart maximum, without losing the resolution of the image 
• Fetal heart color Doppler: Increase the PRF of the Doppler ultrasound maximum possible until color Doppler flow is just right and filling out the cardiac ventricles and aorta. 

In the image below, color gain is too high or PRF is too low to get a good flow study of fetal heart:
